Certificate of Completion Request

Important Information

  • Do not fill out the form until grades are posted.
  • You will need to check your "Preferred" email of record in order to claim your digital certificate.

Step 1

Before filling out the certificate request form, follow the instructions below to update your name, email and/or mailing address if needed.

Your Name

Your Primary Name on file with SF State will appear on your certificate

Name on File:

Your primary name on file with SF State will appear on your certificate. If you would like a different name to appear on your certificate, please do the following before submitting your request:

  1. Log into your Student Center
  2. Click “Access Student Center” after logging in
  3. Scroll down to the "Personal Information" section
  4. Click “Names”
  5. Choose “Add a new name”
  6. Under "Name Type," choose "Diploma"
  7. Enter your name information
  8. Click “Save”
  9. Log out of your Student Center

Email Address

Instructions to access your digital certificate will be sent to the email address marked preferred

Email Address:

Instructions to securely access your digital certificate will be sent to the email address marked as preferred on your SF State account. To update your preferred email please follow the instructions below before submitting your request:

  1. Log into your Student Center
  2. Click “Access Student Center” after logging in
  3. Scroll down to the “Personal Information” section
  4. Select “Email Addresses" from the dropdown menu
  5. Make sure that you have access to the email address with a checkmark next to it in the column labeled “Preferred”
  6. Add or update your email address, and move the checkmark as needed.
  7. Click “Save”
  8. Click “OK”
  9. Log out of your Student Center

Mailing Address

Your paper certificate will be mailed to your Mailing Address on record

Mailing Address:

Your paper certificate will be mailed to your Mailing Address on record. To update your Mailing Address, please do the following before submitting your request:

  1. Log into your Student Center
  2. Click “Access Student Center” after logging in
  3. Scroll down to the "Personal Information" section
  4. Click “Mailing Address”
  5. Click the pencil icon to edit
  6. Click “OK” after editing
  7. Click “Save”
  8. Click “OK”
  9. Log out of your Student Center

Step 2

Obtain an electronic copy of your transcript. An unofficial transcript is fine.

Step 3

Select your program below, and complete the certificate request form. If you do not see your program listed below, please contact the appropriate staff for assistance.
