Enrollment Request Form

  • New Students: Create an SF State Login and follow the email instructions to set an SF State Password.
  • Returning Students: If needed, you can look up your SF State ID and reset your SF State Password.
  • The email address you enter in this form must match the one that is in your student record. To check/update your email address, please open the Student Center and navigate to the Personal Information section (bottom of Student Center) before completing this enrollment request.
  • We will work as quickly as possible to enroll you in your desired classes. Forms submitted past 3 p.m. may not be processed until the next business day.
  • Submitting this request does not guarantee enrollment in a class. We will email you if there is a problem with your enrollment.
  • Monitor your Student Center for any successful enrollments.
  • After successful enrollment, Payment is due by the payment due.
